On September 6, 2020, about 0900 eastern daylight time, a Robinson R22 Mariner helicopter, N194HC, was involved in an accident at Page Field (FMY), Fort Myers, Florida. The flight instructor and pilot receiving instruction were not injured. The instructional flight was conducted under the provisions of Title 14 Code of Federal Regulations Part 91.

Both pilots submitted written statements, and their versions of events were consistent throughout. The preflight and engine-start procedures were completed by the checklist with no anomalies noted. As the engine was accelerated to near its operating rpm, each pilot heard a loud "bang." The student leaned out his door and announced that "the tail was hanging off the back of the aircraft." The engine was stopped, and the event was reported to the helicopter's owner and his mechanic.

Examination of photographs revealed damage to the tailrotor gearbox, tailrotor blades, and the tailcone's section forward of the tailrotor gearbox. These areas and components were sectioned and forwarded to the NTSB Materials Laboratory in Washington, DC for examination.

DESCRIPTION OF ACCIDENT:  

After landing at Mobile, Alabama, a strong gust of wind blew the aircraft sideways to the left.  The left main landing gear and the nose gear traveled off the concrete runway and into the rain soaked grass.  The PIC steered the aircraft back toward the runway, but the nose wheel hooked the edge of the runway causing it to be wrenched out of position.  Trunnion bolts were sheared.  The nose gear collapsed and the propeller struck the runway.  The aircraft ended up on the runway.  The main landing gear did not collapse.  The damage to the aircraft is limited to the propeller, the engine, the engine mount, the lower engine cowling, the nose gear doors and the nose gear assembly.

DESCRIPTION OF DAMAGES: 

The aircraft was disassembled for recovery. The damage includes but may not be limited to:

All 4 blades on the propeller are bent aft. 

Both nose gear doors are damaged beyond repair. 

The bottom of the lower engine cowling is scuffed. 

There is a small hole in the upper engine cowling. 

Some part of the engine pierced the cowling slightly. 

The trunnion bolts sheared causing the gear to collapse. 

The air conditioning compressor mount is fractured. 

The alternator bracket is broken. 

The aircraft’s engine was removed and sent to Dallas Airmotive for an evaluation and returned in a “dummy build” configuration.

On September 19, 2020, about 1925 Pacific daylight time, a North American B25N, N7946C, was substantially damaged when it was involved in an accident near Stockton, California. One pilot and one passenger sustained serious injuries, and one pilot sustained minor injuries. The airplane was operated as a Title 14 Code of Federal Regulations 91 personal flight.

The pilot reported that while enroute to their destination, fuel pressure for the left engine fluctuated, and the left engine briefly lost power before regaining power. The pilot stated that he was concerned that they may have had a fuel pump failure or something similar and decided to turn the cross-feed valve on. A short time later, about 5 miles from their intended destination, fuel pressure fluctuations were observed in both engines, with both engines intermittently losing and then regaining power. Due to multiple residential areas between their location and the airport, the pilot conducted a 180° turn and initiated an off-airport landing. The pilot stated that during the landing roll, he observed a ditch in front of them and was able to get the airplane airborne briefly to avoid it, however, he was not able to avoid a second large ditch. Subsequently, the airplane struck the second ditch, became airborne, and impacted the ground in a nose low attitude, and all three landing gear collapsed.

Both the left and right engines were separated from their respective attach points, and the fuselage sustained substantial damage. The wreckage was recovered to a secure location.

On July 3, 2022, about 0930 eastern daylight time, a Cub Crafters CC11-160 airplane, N55JV, was substantially damaged when it was involved in an accident near Findlay, Ohio. The commercial pilot was not injured. The airplane was operated by the pilot as a flight test conducted under the provisions of Title 14 Code of Federal Regulations Part 91.

The pilot reported that the airplane had been previously involved in an accident in 2020 (in which he was not the pilot). He purchased the airplane and made extensive repairs to return it to an airworthy condition. This accident flight was the first test flight since the previous accident. The pilot taxied from his property to an adjacent off-airport soybean field. He performed a run-up without issue and operated the electric horizontal trim to a full nose up position as indicated on the primary flight display (PFD), and then moved the trim back down to the takeoff position.

When the trim indicator reached the takeoff position, he initiated the takeoff roll. The airplane accelerated normally, and he moved the control stick aft to rotate, however the control pressure felt “really heavy.” He reported that given that this was the first test flight after the previous accident, he immediately aborted the takeoff. In his effort to slow the airplane, it subsequently nosed over “slowly.” The left wing and empennage sustained substantial damage.

A Federal Aviation Administration inspector examined the airplane after it had been moved from the accident site. The horizontal trim position found at the flight control surface and as indicated on the PFD was consistent with a full nose down setting.

The airplane was retained for further examination.

Analysis

The pilot reported that while taking off from a downhill sloped meadow, the airplane encountered a strong quartering tailwind gust, which stopped the airplane from climbing. The airplane subsequently sank, and the left wing struck the ground. The airplane’s left wing and fuselage sustained substantial damage. The pilot reported that there were no preaccident mechanical failures or malfunctions that would have precluded normal operation. The pilot reported that the accident could have been prevented if he had selected a better takeoff site with more options in case of unexpected adverse conditions.
