Collectibles are listed in each stage in the order you get them (not grouped by type). While it would make sense to group collectibles by type, only fools enjoy wasting time watching a loading screen.

You ride a work scaffold/platform down to the street a little bit after starting the mission. When you crash-land, check to see the building you fell off is behind you. Head left and make a second left into the alley. The item you want is on the metal stairs.

Check the street west of the church (use the compass on the mini-map to determine your direction). The camera is set up to catch you sprinting downhill.

At the church's front door, there are some supply boxes piled high with the item on it. Simply walk up to them and locate the item. You may want to neutralize the enemies in the courtyard first, or have a lot of cloaking energy to avoid conflict.

This PDA is in the orange communications trailer in the church courtyard. To unlock the door, you need to first cause the distraction and sabotage both ammunition dumps. Once that occurs, the trailer is accessible (because you need to go inside for an objective). You can download the PDA during that time.
