A day to remember
It was just an ordinary pre-call day that I intended to gather the only few friends of mine in Bintulu for a simple dinner. Ernest and I planned it on a Sunday despite having his post-call fatigue since I was oncall on my birthday. So I invited my landlady first. She questioned straight away "Whose birthday?"
Hence, the secret unveiled. Instead of giving me a surprise, she cutely ordered a birthday cake and asked me to collect from the bakery before dinner. Unpredictably, when we picked them from the doorstep, all the other friends of mine were all gorgeously dressed up with make up while I was only in my routine dress and slippers!
It wasn't an ordinary dinner there after. We had western cuisine followed by the long missing K-session with the most oldies I'd ever sang in the short 2-hours! There I realized singing K isn't just the youngsters' activity.

God is good, all the time
I experience God's faithfulness from day to day. Even when I am down in the dungeon with the worst things in the world happening to me. His mercy never come to an end.
Today, I praise God for two significant events. Firstly, the need to jump start Ernest's car. We always car pool and for only once in a blue moon we drove two cars to work today so the engine was immediately restarted when needed.
Secondly, Ernest has passed MRCS Part 1! He is so happy and danced the Gangnam style! His hard works and weight losing got paid off.
If we look into our heart, there are many things to praise God for. My faithful God, thank You for everything! And all the time, You are good!

Leng's graduation
God is faithful. By His grace, she walked through her rough rides and graduated with degree in hospitality and bussiness management. I feel thankful even to watch my sister's graduation ceremony via live streaming.
If Dato' Lee Chong Wei's Olympic match brought the whole Malaysia together, this graduation ceremony has definitely brought our whole family together. Congratulations once again, my dearest sister! We are so proud of you!

The denture
Accompanied my land lady to get her denture done this fine morning. There hide a classic denture maker in Bintulu town with furnitures easily age a decade. Enjoy the pictures, after trying out what my photoshop can do!

Another new hair do

Gawai Dayak festival
Every first of June is Gawai-Dayak festival, a celebration of the Ibans and Bidayuhs. It is a public holiday in Sarawak and I am blissfully having half day off today!
The families are friendly, hospitable and treated us like no strangers eventhough it was our first meet. Let the pictures speak its colours and culture.

The cause of death
It was a missed miscarriage of a 15-weeker with the fetal parameters at only 12 weeks size, showing either an arrest or shrinkage. Note how its cord tightly entangled around the neck, multiple times till the blood supply was depleted. Hence the fetal demise.

First public forum
It was by chance, that my boss was invited to an event organized by St. Thomas Church to give a talk on breast cancer and cervical cancer. Being a Muslim, she passed the task to the only available non-Muslim in the department - yours truly.
It was only a two-and-a-half-days short notice. I was all stressed up, knowing that being an obsessive compulsive maniac, I would definitely need more time to prepare. Moreover, the stipulated time for the talk was 2 hours! How much do I need to prepare to talk for 2 hours?! I had been praying, for impartation of the message, at least to create self awareness for both breast and cervical cancer.
So today, without familiarizing the crowd before hand, I arrived at destination 10 minutes before time. It was a women fellowship with roughly a hundred of them. They had started praise and worship. With warm welcome, I was ushered down the aisle. The first question directed to me by the chair person was:
"Doktor caku Iban?" (Does doctor speak Iban?)
"Minta maaf. Aku nak nemu caku Iban..." (Sorry, I do not know how to speak Iban...) I answered with a head shake but agreed to deliver my presentation in Bahasa Melayu even though my slides were English written. In my heart, I was a tad worried as to speak Bahasa is fine, but to deliver a formal talk, it would take time to phrase and translate.
I must really thank God that the session went well with good response.
Thank God for the opportunity, for the experience to worship in Bahasa Iban, and for the attentive group who did raise up a lot of questions that showed they really concern.
Also, thanks to the deformed breast props I borrowed from the hospital. It stayed the crowd alert for some hands-on and of course, consuming time a little longer.

New friend
I was just boosting that my laptop is 7 years old and still functioning! But to wait for it to start up takes forever, so as to load a webpage or download. So I passed it to Ernest to bring home for reformat, hoping that it would speed.
He came back and said: "I've got you a new friend!"

Babies are gift from God
A Para 2 was referred to Specialist Antenatal Clinic at 24 weeks of gestation for fetal ascites. She was otherwise well. Transabdominal ultrasound scan revealed a singleton baby with parameters corresponded to date, but gross fetal ascites present. She was suggested to consult fetomaternal specialist in tertiary hospital with referral letter attached. However, she preferred to seek opinion from private sector first and isn't keen to follow up with us again.
Barely 3 weeks from her visit, I tried to call to review her well being. Successful only after a few attempts, she denied who she was initially. I got to know that she had aborted the baby, even before fetomaternal specialist consultation. Was a tad disappointed.
The baby deserves a chance to live, or at least further fetal well being assessment.

The killer embolus
We'd lost another mother lately, a Gravida 7 in mid thirties, expecting her fourth child with anterior placenta praevia type 4. An emergency caesarean section was performed for bleeding praevia at 35 weeks gestation. She collapsed on table shortly after baby was delivered.
She went under general anaesthesia. First noted a drop in end tidal volume followed by circulatory collapse with cyanosis, then DIVC with bleeding from multiple orifices. CPR of more than 2 hours commenced but couldn't beat her death to massive pulmonary embolism, evidenced by the clips recorded by anaesthetist.

1) Foreign body, the embolus in right atrium
2) Enlarging embolus, behaving malignant.
3) Multiple emboli, flopping up and down.
Infraclavicle view, atria on the right and ventricles on the left.
4) Persistent emboli, prior to pulseless electrical activity. Right atrium and ventricle dilatation seen.
5) Acute right heart failure, note some emboli had gone out of atrium.
6) Dislodge of main bulk of emboli, with some small ones still visualized in right atrium.
Pregnancy posts 10 times higher risk of embolism compared to normal population. To undergo a caesarean section increases the risk up to 20%. It was tragic to have lost a mother who was previously so well to massive emboli. Her little daughter remained not cuddled.

Twin!
Dragged myself for locum today. Can't content the happiness picking up a twin pregnancy from a patient who complained of nausea for the past 2 days but did 3 self-done-UPTs which returned negative for the same period of time.
