The Australian Physiotherapy Association is calling on Tasmanian political parties to prioritise healthcare reform, highlighting the need to address patient wait times and intervene for the needs of the population across the state ahead of the upcoming election. APA National President Scott Willis GAICD has worked in regional Tasmania for over 30 years, he says the state’s healthcare model needs to evolve to better meet the needs of its population. Funding physiotherapy services is part of this evolution. 'We have the oldest population in the country, the number of Tasmanians over the age of 85 will nearly double over the next decade. We also have the highest consumption of prescription opioids and incidence of back problems and osteoporosis. New investment in publicly funded physiotherapy will advance health, improve care and increase value for Tasmanians.'   The APA is calling for greater access to physiotherapy services for all Tasmanians, particularly priority populations that stand to benefit the most from physiotherapy to support prevention and maintenance of chronic conditions. AI-Based Follow-Up Tools as a Public Health Instrument In a nation-wide Scottish study by McQueenie (2019), encompassing 15% of the Scottish population, missed follow-ups were associated with a dramatic increase in mortality. Even with 1-3 missed appointments, there was a 1.5 times increase in all-cause mortality compared to regular attenders [1]. A large proportion of these no-shows are due to socio-economic constraints [2]. However, field research studies suggest that "a practical reminder system significantly increases patient attendance at medical outpatient clinics. An intervention focused on specific patient characteristics could further increase the effectiveness of appointment reminders" [3]. That’s why we’ve developed our Follow-Up module for EHR records. The module reads the mass disseminated patients’ records, aggregates their recommendations in a set of time-bound follow-up visit reminders, that can be delivered to the patient in an omnichannel format in accordance with specific guidelines of hospitals and community services. The Peterson Health Technology Institute just published a fascinating article (below) showing that first-generation virtual Musculoskeletal (MSK) care solutions have proven clinical efficacy. On July 1, we're releasing Medbridge Pathways, a next-generation hybrid MSK platform for patients and their local providers that allows virtual and physical care to be intertwined. This approach to care builds on the pioneering work done by first-gen virtual MSK solutions and addresses their three biggest shortcomings: 1. Dis-intermediated local providers - first-gen solutions have done an end-run around existing brick-and mortar providers to build their own digitally-siloed provider networks, leading to gaps in care and generally low patient adoption. 2. Fragmented solution ecosystems - the first-gen landscape can be segmented into 3 distinct segments that deliver a narrow set of services appropriate only for a subset of patients. And when physical care is needed, they can't provide it. 3. Unnecessary costly services - because of point (2), too many first-gen providers have built redundant, human-powered services that don't take advantage of the provider ecosystem that ALREADY exists in the brick and mortar world. While these first-gen solutions have done pioneering work proving that virtual MSK CAN improve patient access and outcomes, their success is limited by the three factors above. Which is why we've designed Pathways differently. First off, we've built Pathways as platform for use by existing brick-and-mortar providers to give them more options and greater reach when treating their patients. Second, because of our clinician-first approach, Pathways can be used to support patients across the acuity spectrum: it can provide low/no-touch, automated care for low acuity patients; it can be prescribed by treating PTs to supplement patient care in between brick-and-mortar visits; it can be used by a hospital's virtual team as a platform that integrates with their care...and it allows any provider using it to bill for RTM. Finally, because we work with - not against - local clinical networks, our solution is significantly less expensive than the alternatives even as it offers greater capabilities to patients and providers alike. If Caroline Pearson - executive director of the Peterson Health Technology Institute and the author of this report's intro - had seen one of our Pathways-enabled clinicians after breaking her leg in 2023, she would have had a very different care experience than what she described. If her provider had had Pathways, Caroline would have had no missing sessions due to work travel, no squeezing visits into inconvenient appointment slots, and no disconnects in care between a first-gen virtual MSK solution and her own local care team. There is a growing consensus of reserch published around the postive impact of First Contact Practitioner Physiotherapists 🙌🏽 The recent GP DES highlights the importance of patient feeback, audit, clinical outcomes and services improvements and with upto a third of GP appointments being for an MSK issue, analysing the impact is essential 👍 This great 2024 paper and well worth a read if you work in #primaycare. The study had 426 participants and looked at several different factors and impacts. Please do take the time to read it. Here is my interpretation and brief summary points: Key Benefits of First Contact Physiotherapy Practitioners (FCPP) in General Practice: ✅ Expert MSKD Skills Access: FCPPs provide specialised expertise in managing musculoskeletal disorders (MSKDs), addressing up to a third of GP workload related to these conditions ⚖️. ✅ Safe and Effective Alternative: FCPP-led consultations are proven to be safe, clinically effective, and cost-beneficial compared to traditional GP-led care 🤝. ✅ Reduced Medication Usage: FCPP consultations result in significantly lower prescription rates, especially for opioids 💊, contributing to better patient outcomes and decreased reliance on medication 🙌🏽. ✅ Quicker Patient Improvement: Patients consulting FCPPs experience faster improvement, reducing the duration of illness and enhancing overall quality of life 💪. ✅ Cost Savings: Embedding FCPPs as a standard model in general practice leads to substantial cost savings for both patients and the healthcare system, making it a financially viable option 💷😊. ✅ Reduced GP Consultations: With FCPPs available, there is a reduction in the number of MSKD consultations with GPs, alleviating the workload pressure on primary care physicians 😌.
