This answer is a mix of experience, practice, biological limitations, and whether the others in this situation are also out of pitch. It is best to follow the lead and the music so that it saves any issue of being way off. I miss the old days of harmonizing without the need for overwhelming pitch correction. I noticed that they are starting to use some of this live as well. I agree that one should match volume, phrasing, and timbre to match the group/situation. Practicing with the piano is also a good method. I use my guitar, as well.

This has several answers depending on the situation. If it is related to a group that is practicing together, one should learn the song structure and chord progressions and then work this out with each member of the group. Depending on the feel of the song and the effect you are looking for, you may pick a root, 3rd or 4th harmony, etc. If the situation is an improv/live on the spot scenerio, then working it out in one's head from the beginning of the tune is important. I call it internal humming. LOL That will help find the right harmony/pitch. It is important to practice these scenarios. Harmonizing in the studio can be a mix of both of these. Preparation and practice.

This is much to say about limitations based on biology and technique. These are both important subject matters related to voice, delivery, and vocal health. Moving smoothly between chest, head, and falsetto takes practice. So do practice for different scenarios. It will serve you well.
