How can Case Managers collaborate to reduce readmissions?

I believe that trust is not a resource that is automatically created between people. I understand that trust is built over time based on experiences and is reaffirmed especially in difficult situations. Mutual support reinforces trust and the entire team acts in a more synergistic, harmonious way and with better results. Positivity, resilience, transparency, flexibility, communication are important ingredients for trust. However, trust ends up being shaken when practical attitudes distance themselves from theory and especially leaders act in a divergent way that they propagate. Leadership must serve as an example of conduct so that the work environment is fertile ground for developing trust.

We can appoint couple of Health care Assistants at every GP care, to call high risk patients on 1,2,3 weekly basis & just ask one simple question,"how are you" to our high risk group of patients & type patient's answers as it is, that can flag up any red flags and alert our clinicians, who doesn't necessarily need to worry about every single patient yet remain involved, until they are comfortable to discharge the patient. This will surely promise 5 star rating for GP surgeries. The only thing we are missing is, 'LISTENING' at the clinicians end. It's high time to self reflect our own practice to gain our patient's faith in our work. Simple measures but doable. Ask yourself, would you like to hear from a robot or a human, when need support?

Feedback is a common practice in everyday life. They occur with the intention of highlighting strengths and opportunities for improvement in workers’ performance. A very important factor to consider in feedback is to try to exemplify the statements that were reported with facts that occurred. Another relevant point: when there is praise, feedback can be public; however, when there is criticism, the conversation must be private, not unnecessarily exposing the employee.

Most organizations have a defined feedback structure, with a standardized layout. The teams that provide feedback must be trained to do so appropriately. In general, conversations take place 1:1 and also include KPIs. However, it is important that feedback, which aims to promote efficiency, is not a source of reduced engagement. When feedback emphasizes points of weakness that are sensitive to the employee and that they have difficulty dealing with, it may be necessary to seek help from HR professionals to safely address them. The risk of poorly conducted feedback causing more harm than good is relevant.

When addressing health all factors must be taken into consideration. However, there is no point just being aware of the problems if there are no specific procedures to support in case of difficulties. As the saying goes: If you don't know what to do with the answer, then there's no point in asking. Therefore, the suggestion would be to have defined support flows for each axis relating to health problems, so that the case can be routed towards a solution.

The manager must have the trust of his subordinates so that he can listen to any difficulties that exist, without prejudice, whether from a professional or personal point of view. When talking about worker performance, there is no way to separate personal problems from professional ones. The human being is unique and life is not limited within the company walls. A family man will never perform well at work if his son is experiencing difficulties. We must seek to maintain the well-being of workers and efficiency will come as a consequence.
