We can't consider that there is one single type of projects, there are projects that can make a big difference and for which you need to take huge risk, and the more conservative ones, in which the risk is more known and easy to address. In any case is usually cheaper to fail fast, than to continue trying to find a solution with small steps. I think the risk culture needs to be present in all the levels of an R&D organization.

The Pre-Mortem is done by asking the participants to imagine themselves one year from now in a future where the project/product/initiative has failed horribly.
