Agree, most of the organisations fail here as they hound the person who detects the problem. Organisations need to create psychological safety for employees to bring problems early. One, early problems detection can save a lot of spent costs on continuing with the problem for longer duration Second, a solution can be found only when the problem is identified. I have seen incompetent managers shouting " come with solutions, don't come with problems" thereby shutting down the employees. I would go ahead with rewarding employees for bringing out problems early, look at causes and solutions, and create a learning module for others to avoid creating similar problems. Many time team members try to solve the problem in a vacuum and this invariably leads to a one side review on the problem and as a consequence the solution is not adequate.
