First we need to be present during handovers and observe what is going wrong, listen to people involved in Shift Handovers activity and ask then how as a management we can help them to smooth Handover practices, it has to be max 5 min, we can utilize DMO tools which can help result recording of past shift handovers, usually workers like easily understandable content for any activity. Documented Shift handover is a necessity for safe process plant operation. There have been several learnings like Piper Alfa where it has been observed that the catastrophic incidents have occurred due to the shift information not properly handed over or poor shift handover. Documenting few major points are mandatory for Shift handover by a shift incharge namely following: 1. Closure status of permits 2. Status and valve position of open jobs 3. Documenting any non routine operation 4. Status and availability of active fire protection 5. And at the last observation of the shift end field round, I want to add that a responsible shift incharge always takes minimum two rounds one at the start of shift and one an hour before the shift end.

Just to add, a well written handover logbook is very essential in safe plant operations. Also, verbal confirmation just incase any vital information was omitted.

Paper log books although outdated but essential for recording daily incidents during the shift. We face problems like writing same opened tasks or notes over and over every day, and once missed by a shift, information becomes misleading. I found electronic logbooks or shift handover software is a useful tool to make seamless workflow easier and functional.

Formality in the turnover process is a must. There are many ways to establish this formality, but it must result in the clear acknowledgement of what is being turned over. In my experience this is best completed by a face to face turnover with all equivalent parties, followed by a discussion of the results of that turnover with the oncoming team as a group. This allows any questions to be raised and addressed by the person in charge. If that person cannot answer the question, it can be addressed by consulting with the higher authority. Many questions can be avoided with clear guidance left by management to cover the back shift(s) (i.e., night orders) that delineate the expectations for the night.
